Making Every Adult Matter (MEAM) Officer

Location: Buckinghamshire County Wide

Overview

Do you want a role where you can make a genuine difference to people facing some of life's most complex challenges? As a MEAM Officer, you'll support adults experiencing multiple disadvantage to build confidence, overcome barriers and access the services and opportunities they need to thrive.

Joining Buckinghamshire Council's Adults & Health service, you'll become part of the dedicated Making Every Adult Matter (MEAM) team, delivering intensive, person-centred support to individuals facing challenges such as homelessness, mental health issues, social isolation, and substance use.

This is a 12 month Fixed Term role

About us

Buckinghamshire Council is a relatively new organisation that combines the strengths and expertise of the previous five councils in the county to create positive change for local people, communities, and businesses. We are firmly focused on the future and have made a clear commitment to serve residents better and to provide stronger representation for Buckinghamshire, both locally and nationally.

About the role

This is a rewarding and varied role where no two days are the same. You'll work closely with individuals experiencing multiple disadvantages, helping them take control of their lives through tailored support, practical guidance and strong advocacy. Using a trauma-informed and strengths-based approach, you'll develop trusted relationships, co-produce personalised support plans and help people navigate complex systems and services.

Working as part of a multi-agency network, you'll collaborate with a wide range of internal teams, community organisations, healthcare professionals and specialist support services. You'll identify gaps in provision, influence positive change and contribute to improving outcomes for some of Buckinghamshire's most vulnerable residents. This is an opportunity to be part of an innovative programme that places people at the heart of everything it does, empowering them to build sustainable and positive futures.

Some of your key responsibilities will include:

  • Managing a caseload of individuals experiencing multiple disadvantage and providing intensive, personalised support.
  • Co-producing support and risk management plans that are strengths-based and trauma-informed.
  • Building effective partnerships with local organisations, services, and community groups to improve outcomes for individuals.
  • Maintaining accurate records and contributing to service development and continuous improvement.

On-the-go:

Our ‘on-the-go’ roles mean you’ll usually start and finish your day from a base, moving around between different locations throughout the day. You won’t need a fixed desk to work from, but you’ll have access to our network where you can keep up to date with admin and the latest staff updates.

About you

We're looking for a compassionate, resilient and proactive individual who is passionate about supporting people to achieve positive change. You'll be someone who can build trust quickly, communicate effectively with a wide range of people and remain calm and professional, when managing challenging situations. A collaborative approach is essential, as you'll be working closely with colleagues and partner agencies to deliver the best possible outcomes.

To be successful in this role, you'll have experience working within a relevant support, health, social care, community, or customer-focused environment, alongside a strong understanding of partnership working and person-centred approaches. You'll be confident managing complex caseloads, maintaining detailed records and using your influencing and communication skills to support individuals with diverse needs. A qualification at NVQ Level 4, degree level, or equivalent is essential, and a full UK driving licence is required. Experience of trauma-informed practice, strengths-based approaches and working with vulnerable adults would be highly beneficial.

If you're passionate about empowering people, building stronger communities and making a lasting impact, we'd love to hear from you.
